Write down where the work started, which systems held the information, who touched it, and what happened next. Use the actual path, including the waiting.
Measure elapsed time, manual steps, mistakes, or missed handoffs. Use records where they exist. Mark anything you do not know as unknown.
Pick the smallest change that can improve the outcome. Define the information it needs and the decisions a person should keep.
Practice with normal requests, missing information, and an exception. Let the person who does the work decide whether the result is useful.
